It is separate from our Notice of Privacy Practices, which governs Protected Health Information (PHI) in your medical record under HIPAA and other applicable law. Where the two overlap, the Notice of Privacy Practices controls your medical record.

This policy covers mydfwdoctor.com and any form you submit through it. It does not cover the secure Patient Portal or the online booking service, which are operated by our electronic health record and scheduling vendors under their own privacy terms and our business associate agreements with them.

We collect the information you choose to send us when you:
- Submit the contact form or request an appointment
- Ask a question about services, membership or insurance
- Communicate with the office by email, phone or text message
This is typically your name, email address, phone number, preferred location and the reason you are getting in touch. Please send only what is needed to reach you — see the notice above about Protected Health Information.
When you use the online booking flow, we also keep a record of your booking acknowledgement: the name you typed, the date, any signature you chose to draw, your contact details, and the version of the booking terms you agreed to. This record exists so both you and the practice can show what was agreed; it contains nothing about your health.
Automatically Collected Information
When you visit the website we record limited technical data on our own servers: your IP address, browser type, the pages you view and the referring URL. This is used to understand site traffic and improve the experience. It is not used to build an advertising profile.

First-Party Analytics
We operate our own first-party analytics. When you visit, we record limited technical data — your IP address, browser type, the pages you view, and the referring URL — on our own servers to understand site traffic and improve the experience. This information is not sold and is not shared with third-party advertising networks.
Spam & Security
Forms on this site may be protected by Cloudflare Turnstile, a bot-detection service that processes technical signals from your browser to distinguish real visitors from automated abuse. It loads only on pages that contain a protected form.
Third-Party Analytics & Advertising
We do not load Google Analytics, Google Tag Manager, the Meta (Facebook) Pixel, or other third-party advertising or tracking technologies on this website. Pages about symptoms and conditions can reveal a great deal about a person, and we do not hand that browsing to an advertising network. Only the strictly necessary, first-party tools described above are used.
Two infrastructure services do load from third-party servers: web fonts from Google Fonts and two animation libraries from the jsDelivr content-delivery network. Fetching these files discloses your IP address and this website's name — but never which page you are reading — to those providers. Neither sets cookies or tracks you.
Your Choices & State Privacy Rights
Because no third-party advertising or analytics technologies load here, there is nothing to opt out of beyond your browser's own controls, which you can use to block or delete cookies at any time. If you are a Texas resident, the Texas Data Privacy and Security Act gives you the right to confirm, access, correct, delete and obtain a copy of your personal data, and to opt out of targeted advertising, sale and profiling. If you are a California resident, the CCPA gives you comparable rights and the right to opt out of the “sale” or “sharing” of your personal information. 9. SMS Terms of Service
SMS consent and your phone number
The phone number you give us as part of the SMS consent process, and your consent itself, are never shared with third parties or affiliates for marketing purposes. We do not sell, rent or trade mobile numbers.
Types of messages you may receive
If you have consented to receive text messages from My DFW Doctor, you may receive:
- Appointment reminders and confirmations
- Vaccination and preventive care reminders
- Follow-up messages about your care
- Billing and account inquiries
For example: “Hello, this is a friendly reminder of your upcoming appointment with My DFW Doctor on [date] at [time]. Reply STOP to opt out of SMS messaging at any time.”
Some of these messages are promotional — a request for a review, or information about a service such as our membership. We never text you on behalf of anyone else, and we never sell or pass your number to another company.
Message frequency
Message frequency varies with the type of communication. You may receive up to 2 messages per week about your appointments or your account.
Message and data rates
Standard message and data rates may apply, depending on your carrier's plan. Rates may differ for messages sent domestically and internationally.
How to opt in
You may consent to receive text messages from My DFW Doctor by ticking the SMS consent box on a form on this website, by completing a paper form at the office, or verbally during a conversation with our staff.
How to opt out
You can opt out at any time by replying STOP to any message you receive. You can also contact the office directly and ask to be removed from our messaging list.
